Oathkeeper is a spirit residing in 
Lady Geist's arm that provides her with power and a youthful appearance in return for feeding on the life force of others.

Summary
Circa 1899, the first Maelstrom happened, bringing various supernatural forces to Earth. Around this time, Lady Geist was an old lady stuck in a nursing home, dreaming of her glorious past. At this moment, Oathkeeper revealed himself to Geist with a proposition: he could restore her youth; all she had to do was drain the essence of the living to sustain herself.
Understanding that Oathkeeper likely couldn't be trusted, Lady Geist had a ward built to bind Oathkeeper to her arm shortly after their deal was made. The purpose of the ward was to limit Oathkeeper's influence over Geist, keeping him under control. As the ward was "against the spirit of the deal," Oathkeeper promised to make Geist suffer if he ever broke free. Since that day, Oathkeeper has been binded to Geist for roughly 50 years.
The ward binding Oathkeeper was originally constructed by Lady Geist's son, Walter Geist. The resposibilities of maintaining and upgrading the ward after Walter's death fell to his son, Johann Geist Jr. Aside from keeping Oathkeeper under control, the ward also prevents Oathkeeper from interacting with the world outside his prison, and only Lady Geist can hear him speak. Lady Geist can choose when she wants Oathkeeper to see or listen to the outside world.[1]
Drifter knows Oathkeeper, claiming to go "way back" with him. With Abrams' ancient Tome being used to summon Oathkeeper to Earth in the first place, it's implied that Oathkeeper existed on Earth long before the First Maelstrom.
Oathkeeper has been bound by a ward on Geist's arm, but the ward is nearing the point at which it can no longer contain Oathkeeper.
Lady Geist's visual novel, The Binding of Oathkeeper, tells of Lady Geist's story to prevent Oathkeeper from breaking free and killing her.
Oathkeeper seems to originate from a ripped page in the tome that 
Detective Abrams carries.
It is possible for Oathkeeper to be transferred to another person, this is shown in the novel where one of the options is giving him to Maximilian Fairfax. He is described as a "parasite" and can't survive for long without a host.
Lady Geist is performing the ritual in order to ask the Patron for a solution to Oathkeeper.
